Please be sure to fill out the Vendor Bid Authorization Sheet completely to include all pertinent information on the left hand side and your vendor information on the right hand side.
Also be sure to review the attached 2017-2018 Bid Conditions and Instructions, where the requirements for submission are detailed. The key features are that the authorization page must accompany a bid submission, along with a bid security bond as outlined.
