This is the image for the news article titled Out-of-State Travel RequirementsTo comply with a new order enacted by Dr. Rachel Levine of the Pennsylvania Department of Health on November 17 and effective immediately and until further notice, parents/guardians of students traveling outside of Pennsylvania must complete this form notifying the South Park School District PRIOR TO leaving Pennsylvania. After completing this form, your child's school principal and nurse will be notified. Additionally, for a student to return to school after traveling outside of Pennsylvania, he/she will need to provide documentation to his/her school principal or school nurse of a negative COVID-19 test within 72 hours PRIOR TO entering Pennsylvania or will need to quarantine for 14 days upon return to Pennsylvania. This requirement takes effect on Friday, November 20 and continues until further notice. This order does not apply to anyone who commutes to and from another state for work or medical treatment.
