South Park Athletics Physical Registration is now open to all sports for the 2024/25 school year. A physical that is dated on May 1, 2024 or after is eligible for their physical registration. All physicals must be registered online using the link below. If you registered last school year, your same login and password will work, but will need to update your new physical. Attached is the Section 6 CIPPE Form that must be filled out by your physician, and uploaded when registering your physical. Please note, that NO PAPER COPIES will be accepted.
If you are a multi-sport student-athlete, please register your physical with the first sport that you are playing this school year. Example: If you play soccer and track/field, please register your initial physical for soccer, then you will do the recertification for track/field in the spring.
